Funding Allocation

Please fill out this form only for distribution or reimbursement of PTO funds, which have been allocated at a general membership meeting.  Use the Funding Request Form if you are requesting Funds for a new program or purchase.  This form will be directly submitted to the PTO Treasurer, Kristi Herbrechtsmeyer.  If you have any questions please contact  her at: 651-784-7693.
